Healthy Connections Wigtownshire is a new programme now available to patients from Cairnsmore, Merrick, Wigtown and Whithorn medical practices.
Sometimes it may be referred to as social prescribing as it is linking those attending GP consultations with wider supports and services available within the local community to improve health outcomes.
Healthy Connections Wigtownshire is for people who are aged 18 and over, vulnerable and at risk groups such as low income single parents, recently bereaved elderly people, people with long term conditions and people with severe, enduring mental health problems and for people who frequently attend primary care.
Healthy Connections offers access opportunities for patients to improve their own health and wellbeing.
